Outstanding Alumnus Nomination Form

Jefferson State is now accepting nominations for the college’s 2025 Outstanding Alumnus Award. To make a nomination, please complete the information below. The deadline for nominations is March 13, 2025.

Who can be nominated?
Anyone who has taken any class at Jefferson State. Nominees do not have to have a degree from Jefferson State.

Who can submit a nomination?
Anyone.

Criteria for selection
The nominees will be evaluated on any of the following: career achievements, community service,  personal accomplishments,  or contributions to enrich the lives of others.
